SNAP has a long and substantial history of successfully developing
and managing collaborative projects for Suffolk County.  Since
1986, SNAP has provided long-term leadership, program and fiscal
oversight on five state funded consortium grants, funded through
the New York State Department of Health (CBAPPP) and the New
York State Office of Children and Family Services (APPS and
Advantage After School), subcontracting with more than twenty local
agencies in high risk communities throughout Suffolk, including
Amityville, Wyandanch, Central Islip, Bay Shore, Brentwood,
Bellport, West Hampton Beach, Riverhead, Mastic/Shirley and
Patchogue/Medford. In addition, SNAP receives funding from the
AIDS Institute and contracts for services directly with several local
school districts.  Together with our subcontractor partners we offer
young people and their families a range of services to help them
develop the necessary support systems for a trouble free
adolescence and healthy adulthood.
